SHINGRIX UPDATE: Over 5,500 Shingrix Shots have been administered or dispensed by our fast, friendly and professional team of pharmacists and nurses.  If you are on a waiting list somewhere and would like to be protected from Shingles NOW, Weinstein Pharmacy has an immediate supply on hand and can assist.

Effective November 12, 2019.  Shingrix immunizations will be offered on a first come, first serve basis at our pharmacy on MONDAYS, TUESDAYS, WEDNESDAYS, THURSDAYS, and FRIDAYS from 9am to 2pm (except holidays).  We do not accept appointments or hold vaccines.

The CDC has sent a notice, effective October 2018, informing the public that the second dose of Shingrix may be received after the 6 month mark. It is not necessary to re-start the series. (Please see attached memo from GSK).

 

For Medicare Advantage Members, Shingrix is almost always covered under your Part D Plan. We will not know how much your copay is until we process your claim at our pharmacy during your visit, as this may change from day to day, and depending if you have met your annual deductible. 

 

For patients under 64 years old and without Medicare, we participate with HMAA, HMSA, PSWA, UHA, & TRICARE. We will try to verify your benefits online during your visit, but if you would like to know more prior to your visit, please contact your insurance carrier for details. HMO members are required to obtain a prescription from their in-network provider. If you have a PPO plan, we are able to provide the order for you. If you are unsure what type of plan you have, please contact your insurance carrier, as we do not know unless we verify your information during your visit.

 

To expedite your visit, please complete the attached Patient Immunization Consent Form.  All fields are required.

Please be sure to also bring in your State ID, all current medical and drug insurance cards, and money for any co-pays.
